An observational study of 35 questions phrased from the perspective of an international patient, measured across ChatGPT, Gemini, Perplexity and Meta AI. The results describe visibility observed in this panel at the collection date; they do not measure medical quality, safety or clinical outcomes.
On the full grid, Safe Fertility Center (KVS 100) clearly leads Bumrungrad (79.4): the reference specialist clinic passes the default hospital reflex. Behind them, Bangkok Hospital (50.9) and Prime (41.9) form a second tier — and Jetanin (41.3) jumps to 5th, carried by the general queries. The score differences narrow considerably across the rest of the ranking.
On procedure queries (ICSI, egg freezing, FET…), it's Prime Fertility — a clinic, not a hospital — that leads. In this panel, procedure-specific queries favour some specialist providers more strongly than general queries. This association should be tested over time; it does not, by itself, establish causation.
Genea / GenPrime, the Australian-rooted brand, is cited 18 times across all 4 engines (KVS 17.8) — right into the Australian queries where its "RTAC" DNA resonates. For brands with similar names, this pattern warrants monitoring for possible entity confusion in generated answers.
All five legal questions (foreigners, single women, same-sex couples, surrogacy, sex selection) are ghost or near-empty — two with no clinic cited anywhere, three with at most 3 clinics — while Konkai and PlacidWay lurk in the answers. In this measurement, these questions contain relatively few direct references to fertility providers.
27 % of the sources Perplexity cites (121 of 443) are portals and listicles — Bookimed, PlacidWay, Konkai, CleverThai… and ChatGPT leans on the same middlemen (MediGence, Best Clinic Abroad, Infertility Aide). Even when a clinic is named, a portal often supplied the cited material. This highlights the potential importance of third-party sources in retrieval, without demonstrating that any single source causes a recommendation.
Relative index out of 100 combining citation rank, appearance frequency and multi-engine coverage, then normalised to the leader of this study. KVS is not a probability of recommendation, market share or measure of medical quality.

Dots = ChatGPT · Gemini · Perplexity · Meta AI (filled = cited at least once). Red rows = aggregators capturing citations in place of clinics. Gold row = Genesis Fertility Center (GFC): 4 citations across the 140 question × engine cells, never in the top three.
A global ranking hides an important point: each family of questions has a different competitive structure, with more stable leadership in some areas and low citation density in others.
Safe × Bumrungrad duopoly by volume — no engine agrees on the #1
Specialisation beats size — Prime leads
Bumrungrad ahead — but ChatGPT cites even small clinics (BIC #1 on one query)
Nearly empty — a hospital wins on its published packages
Safe leads, but leadership remains fragmented
All 5 queries ghost or near-empty — only aggregators lurk here
Long answers, almost no clinic named
Some source-market associations are visible, but leadership remains fragmented
Bumrungrad leads the English-speaking-doctor query, while coverage remains split across engines
Repeated appearance across four engines can reinforce perceived familiarity, although this study does not measure patient behaviour. Only 9 clinics out of 34 make the clean sweep: Safe Fertility Center · Bumrungrad Fertility & IVF Clinic · Bangkok Hospital Fertility Center · Prime Fertility Center · Jetanin Institute · Samitivej Fertility Center · VFC Center (Vejthani) · Genea / GenPrime Genea · Deep & Harmonicare IVF Center. At the other end, 5 clinics exist on a single engine only. Case in point: Genesis Fertility Center — 4 citations out of 140, never in the top three, consistently in position 5 when ChatGPT mentions it. Being present in one engine is not the same as having broad, stable visibility across the panel.
Protocol. 35 questions phrased in a patient's voice (never "rank the 10 best clinics"), across 9 categories: general, procedures, success rates, price, trust, legal framework, destination comparisons, source markets, logistics. Asked in fresh sessions, web search on, across ChatGPT, Gemini, Perplexity and Meta AI — July 2026.
Limitations. This is an observational study of engine outputs at a specific point in time. It does not establish that a particular page, source or optimisation causes a citation, and it should not be used as a medical recommendation or quality ranking.
KAIS measures AI-search visibility, the sources used by engines, competitor gaps and information risks. The engine panel is adapted to the target market and defined before the engagement. No citation or ranking is guaranteed.
